Problem

  • Unexpected releases of toxic, reactive, or flammable liquids and gases during processes involving hazardous chemicals have been reported for many years, and in various industries
  • There is a potential for an accidental release any time hazardous chemicals are not properly controlled, creating the possibility of disaster

Some of Catastrophic Chemical Incidents –

  • Flixborough, UK , 1974 – demolished by huge explosion
  • Manfredonia, Italy, 1976 – explosion occurred due to Arsenic release
  • Bhopal Gas, India, 1984 – relief valve lifted on a storage Tank
  • Pasadena, TX, 1989 – release occurred on a polyethylene plant
  • Mexico City, Mexico, 1984 – explosion occurred at PEMEX LPG terminal
  • Malaysia, 2020- explosion occurred at the Pengerang refining and petrochemicals

How will PSM Help

  • The Major objective of process safety management of highly hazardous chemicals is to prevent unwanted releases of hazardous chemicals especially into locations which could expose employees and other to serious hazards
  • Process safety management is the proactive identification evaluation and mitigation or prevention of chemical releases that could occur as a result of failure in existing process
  • An effective & mature PSM requires a systematic approach to deal with whole process
  • The Process safety management standard targets highly hazardous chemicals that have the potential cause to catastrophic incident
  • This standard is to aid employers in their efforts to prevent or mitigate chemical releases that could lead to catastrophic in their organization
  • To Control such incidents, employers needs to develop the necessary expertise, experience, judgment and proactive initiative within their workplace
  • OSHA Standard are required to be followed to avoid such incidents
